One tactic used by the mafia is to claim doctor, especially when the first mafia is forced to claim when under pressure of lynch (they need to claim something). It is such a powerful claim that it may be more likely that claimed doctors are mafia than doctor. It's often used to draw out the real doctor. It's a difficult position for the Doctor to be in. Therefore, they often seem somewhat incoherent or indecisive. Someone will definitely not believe a doc claim, but be tight-lipped about why. It's a judgment call, but it's not too tough to spot.
